916 THE SMARTWATCH DETECTS ECG ABNORMALITIES TYPICAL OF BRUGADA SYNDROME

Abstract Introduction Brugada syndrome (BrS) is a genetic disorder with characteristic pattern on the electrocardiogram (ECG) that predisposes to lethal arrhythmias and sudden cardiac death (SCD). Precise diagnosis therefore mandatory. Aims We evaluated feasibility accuracy of smartwatch in recording multiple ECG leads detecting ST-segment changes diagnostic for BrS both basal condition after ajmaline infusion. Materials Methods Twelve-leads ECGs were obtained 32 unselected patients admitted at our institution type II/III elevation suspected from September 2021 March 2022. After written informed consent continuous monitoring during test (1 mg/kg, i.v. 10 min), consecutive strips acquired techniques analyzed before drug administration. For each patient was tested three different sites: wrist, abdomen chest, intercostal spaces second fourth further studied. The concordance among results standard recordings assessed using Cohen κ coefficient Bland-Altman analysis. Results Subjects 42±15 y.o. average; 67% men (n=25), reached 12 cases (37.5%). Concordance found between identification following findings: i) normal/negative (Cohen coefficient, 0.89); ii) shift 0.934). In addition, analysis demonstrated assessment amplitude (bias, −0.003; SD, 0.68; lower limit, −0.52; upper -0.031). power diagnosing normal showed sensitivity 92% (95% CI, 66%-99%) specificity 100% 64%-100%); detection diagnosis, 64%-99%) 95% 76%-99%). Conclusions findings this study suggest agreement multichannel acquisition syndrome-associated changes.
